Screening and Treatment options for Early Esophagogastric cancers

Esophagogastric cancer had different histology and natural history in different geographic regions, and strategies for prevention and treatment are tailored to the histology and previous history of GI disorders. Asian countries with widespread screening have survival rates double that of countries without population screening. Individuals with ALDH2-deficient genotypes have increased risk for esophageal cancers from drinking alcohol. Minimally invasive surgical techniques are discussed.

Risk Factors and Prevention of Colorectal Cancer

Obesity is a major risk factor for 13 cancers, especially colorectal cancer (CRC) and has surpassed the cancer risk of tobacco. CRC is increasing in younger individuals under 45 years of age. There is a multitude of biological mechanisms linking energy balance and obesity as a driver for cancer, also to survival. The gut microbiome is also associated with survival in early-stage CRC.

Overcoming immunotherapy resistance in treating GI cancer

Gastroesophageal and colorectal cancers are a major life-threatening disease and often present at advanced stages. Discussions about selecting high-immunogenecity subtypes (such as MSI hi; high TMB) to optimize chemoimmunotherapy combinations and facilitate organ-sparing operations, and to propose new strategies for improving immunotherapy efficiency in GI cancers by reversing immunotherapy resistance with drugs that induce immunogenic tumor cell death and to modulate the immune microenvironment.

Prevention, screening and treatment options for inherited pancreas cancers

Prevention, screening and treatment options for inherited pancreas cancers

Screening should be considered for individuals with inherited pancreatic cancer predisposition, including those with a strong family history, high-grade intraductal papillary mucinous neoplasm (IPMN), or known genetic syndromes such as Peutz-Jeghers syndrome or hereditary pancreatitis. Particular attention should be given to individuals harboring germline pathogenic variants in genes such as BRCA1/2, PALB2, ATM, or mismatch repair genes associated with Lynch syndrome. Early detection through routine screening in these high-risk individuals has been associated with significantly improved outcomes. For select high-risk patients, prophylactic organ-preserving pancreatic surgery can be safely performed to reduce cancer incidence. A video demonstration of robotic pancreatectomy for this indication is included.

Genetic Testing in healthy Population and the cancer population

Genetic testing for inherited (germline) mutation and pathological variants have become technically and economically feasible. Most cancer patients with possible inherited contribution should undergo germline testing, especially those with breast, ovary, and colorectal cancers. This information ,plus family history, lifestyle, age and gender can be used to make informed for personalized risk stratification in individuals and family members. Population-based genetic screening can identify individuals at risk for future cancer development (often at an earlier age) and can benefit from prevention intervention or screening.

Prevention policies to prevent skin cancer in Australia, United States and Poland

The highest incidence of skin cancer, including melanoma occurs among fair-skinned population in Australia, New Zealand and the United States. Government sponsored nationwide educational programs have been instituted in Australia which now sees a decrease in melanoma incidence. Tanning beds and salons are also associated with melanoma and are attributed as the primary cause of melanoma in young adults. Government policies have banned tanning beds in Australia altogether and an increasing number of states in the USA have prohibited use of tanning beds for those under 18 years of age. The government of Poland now uses this as a model policy in their country.

Immune Regulation and Cancer Immune Therapy

Professor Yin describes his research involving the protein kinase PAK1, which functions as a "master controller of T-cell immunity." PAK1 acts as a promoter of CD8+ T-cell exhaustion and contributes to decreased tumor immunity, making this molecule a potential target for cancer immunotherapy. His research also explores cancer detection through metabolomics combined with artificial intelligence as a screening tool for lung and pancreatic cancers.

Precision prescribing for cancer patients

Pharmacogenomics is an emerging field of genetic testing that identifies individuals who metabolize commonly used drugs—including many cancer therapies—either too rapidly (resulting in reduced efficacy) or too slowly (leading to drug toxicity). Adverse drug reactions are the fourth leading cause of death in the United States, with oncology drugs being a major driver of drug-related toxicity. A Chinese National Genomics Pilot Study involving over 6,000 cancer patients demonstrated that dose adjustments based on individual patient genomes significantly reduced drug toxicity-related hospitalizations and decreased overall hospital costs.
